The friendly was played in the southern Polish city of Kielce in the run-up to the European U21 championships, which will be held in Poland in June.
The Poles dominated the match from the start but had trouble in creating scoring chances.
The first half ended goalless. The Czech Republic’s Vaclav Cerny put his side one up five minutes into the second half.
Jarosław Niezgoda equalised for Poland in the 84th minute, but Ales Cermak secured the winner for the Czechs in extra time.
